| ［摘要］ 目的 探讨创伤性连枷胸的临床特点和机械通气治疗效果。方法 回顾性分析2006-06～2012-05收治的32例创伤性连枷胸患者的临床资料。结果 治愈26例，死亡6例。呼吸机辅助呼吸通气模式主要以同步间歇指令通气（SIMV）+压力支持（PSV）+呼气末正压（PEEP）为主。呼吸机通气前后的动脉血气分析pH值、氧分压（PaO2）、二氧化碳分压（PaCO2）、氧合指数(PaO2/FiO2)明显改善（P<0.05或<0.01）。结论 机械通气能维持患者的呼吸功能和胸壁的稳定性，是治疗创伤性连枷胸的关键。 |

| ［Abstract］ Objective To study the clinical features of traumatic flail chest and the effect of mechanical ventilation on it.Methods A retrospective analysis on the clinical characteristics,diagnostic and therapeutic experience of 32 patients with flail chest was performed in our hospital from June 2006 to May 2012.Results Twenty-six cases were cured,6 cases died. Ventilator-assisted breathing ventilation mode mainly included synchronized intermittent mandatory ventilation(SIMV）,pressure support (PSV） and positive end-expiratory pressure(PEEP).After mechanical ventilation,the arterial blood gas analysis of patients showed pH value,partial pressure of oxygen(PaO2),partial pressure of carbon dioxide(PaCO2) and oxygenation index(PaO2/FiO2) significantly improved compared with those before mechanical ventilation (P<0.05 or <0.01).Conclusion Mechanical ventilation can maintain respiratory function and the stability of the chest wall of patients with traumatic flail chest,which is the key to treatment of traumatic flail chest. |
