HRT-Ⅱ和自动视野计诊断原发性慢性闭角型青光眼指标的相关性研究

摘要:
[摘要] 目的 探讨海德堡视网膜断层扫描仪(HRT-Ⅱ)在原发性慢性闭角型青光眼(CPACG)诊断中的意义。方法 分别用HRT-Ⅱ和自动视野计(Humphrey)对40例(80眼)CPACG患者的视盘和视野进行检测,并对视盘参数与视野平均缺损作相关性分析,统计分析HRT-Ⅱ检测的敏感性。结果 HRT-Ⅱ检测异常74眼,Humphrey检测视野异常72眼,视野异常和HRT-Ⅱ异常符合率为97.30%;Humphrey检测视野正常8眼中HRT-Ⅱ检测异常2眼、正常6眼,HRT-Ⅱ检测正常6眼中Humphrey检测6眼视野均正常。盘沿面积(RA)、盘沿体积(RV)、视杯形态测量(CSM)与平均视野缺损(MD)呈正相关,杯盘面积比(CDAR)与MD呈负相关。结论 HRT-Ⅱ检查敏感性良好,与视野检查有良好的相关性,可以用于CPACG患者视神经损害的监测及早期诊断。

Correlation between optic disc parametes of HRT-Ⅱ and visual field defect of Humphrey detection in primary chronic angle closure glaucoma

Abstract:
[Abstract] Objective To explore the diagnostic significance of the Heidelberg retina tomograph(HRT-Ⅱ) in primary chronic angle-closure glaucoma(CPACG).Methods HRT-Ⅱ and automatic perimeter(Humphrey detection) were used in 40 cases(80 eyes) of CPACG for testing optic disk and visual field, and the correlation between the optic disk parameters and average defect of visual field were analyzed, the statistical analysis of HRT-Ⅱ detection sensitivity was performed.Results Seventy-four eyes were abnormal by HRT-Ⅱ detection, and 72 eyes were abnormal by Humphrey detection, the coincidence rate between Humphrey detection and HRT-Ⅱ detection was 97.30%. In 8 eyes which were normal visual field in Humphrey detection, there were 2 eyes abnormal and 6 eyes were normal in HRT-Ⅱ detection. In 6 eyes which were normal in HRT-Ⅱ detection, there were 6 eyes were normal visual field in Hunphrey detection. The average visual field detects(MD) showed a statistically significant positive with rim area(RA), rim volume(RV), optic cup shape measurement(CSM) and a negative correlation with cup dish area ratio(CDAR).Conclusion HRT-Ⅱ detection has a high sensitivity and a better correlation with Humphrey detection, and can be used for monitoring and early diagnosis of optic nerve damage in patients with CPACG.
