宫颈癌及宫颈上皮瘤变患者Th1/Th2水平变化及其相关炎性因子的检测与临床意义

摘要:
[摘要] 目的 观察比较宫颈癌患者、宫颈上皮瘤变患者(cervical intrepithelium neoplasia,CIN)及健康对照者外周血中Th1/Th2水平变化及相关炎性因子IL-2、IFN-γ、IL-6、IL-4、IL-10的表达水平变化,为宫颈癌及宫颈上皮瘤变的早期诊断提供理论依据。方法 采用酶联免疫吸附(ELISA)法检测40例子宫颈癌患者、40例子宫肌瘤患者及50名健康对照者外周血中Th1细胞因子白细胞介素-2(IL-2)、干扰素-γ(IFN-γ)的表达水平,Th2细胞因子IL-4、IL-6、IL-10的表达水平,流式细胞仪检测Th1/Th2细胞的比值变化。结果 宫颈癌组及宫颈上皮瘤变组与对照组比较,Th2细胞因子IL-4、IL-6、IL-10表达均升高(P<0.05),Th1细胞IL-2、IFN-γ表达均下降(P<0.05),Th1/Th2比值下降(P<0.05)。随着宫颈癌分化程度的降低,Th1细胞因子的降低和Th2细胞因子的升高更加明显(P<0.05)。结论 宫颈癌患者中可见Th1细胞因子降低,Th2细胞因子升高,Th1/Th2比值下降,且与肿瘤分化程度有一定的关系,提示其可能在宫颈癌的发生、发展过程中扮演着重要的角色,对其水平的检测将有助于宫颈癌病情监测。

[Abstract] Objective To detect the expression levels of Th1/Th2 subpopulations and Th1 cytokines interleukin-2(IL-2), interferon-γ(IFN-γ) and Th2 cytokines IL-4, IL-6, IL-10 in the serum of patients with cervical carcinoma, hysteromyoma and the healthy controls, and to provide the basis for the early diagnosis of cervical cancer and cervical intrepithelium neoplasia.Methods The peripheral blood was collected from 40 cases with cervical carcinoma, and other 40 cases with cervical intrepithelium neoplasia and 50 healthy people. Th1 cytokines interleukin-2(IL-2), interferon-γ(IFN-γ) and Th2 cytokines IL-4, IL-6, IL-10 in the serum of all cases were detected using ELISA kit. Flow cytometry was used to detect the ratio of Th1/Th2 subpopulations.Results The expression levels of IL-2, IFN-γ decreased, and IL-4, IL-6, IL-10 increased, the ratio of Th1/Th2 decreased in the cervical cancer group and CIN group compared with those in the control group(P<0.05). Th1 cytokines reduced more and Th2 cytokines rose more in CIN group than those did in the cervical cancer group(P<0.05).Conclusion Th1 cytokines decrease while Th2 cytokines increase, and the ratio of Th1/Th2 decreases in the patients with cervical cancer. The expression levels of Th1 and Th2 are associated with tumor pathological grade, which reveals that Th1/Th2 plays a vital role in the development of cervical carcinoma,and could be helpful to determine and monitor the tumors properly.
