PNF技术联合高压氧治疗脊髓损伤合并不完全性截瘫患者的临床疗效观察

摘要:
[摘要] 目的 观察神经肌肉本体感觉促进(proprioceptive neuromuscular facilitation,PNF)技术联合高压氧治疗脊髓损伤合并不完全性截瘫的临床疗效。方法 选取脊髓损伤合并不完全性截瘫患者48例,均行脊髓减压、脊柱内固定手术治疗,均于手术后1~2周开始进行康复治疗。随机分为A组和B组,每组24例。A组采用PNF技术联合高压氧治疗;B组采用常规运动康复训练治疗及针灸治疗,两组均康复治疗6个月。根据中国脊髓损伤协会评分标准对康复治疗前后的感觉及运动进行评分,根据改良巴氏指数(modified Barthel index,MBI)评定表对日常生活活动能力进行评定。结果 两组患者康复治疗后感觉、运动评分及MBI较治疗前显著提高(P<0.05)。A组康复治疗后感觉、运动评分及MBI增加幅度明显大于B组(P<0.05)。结论 PNF技术联合高压氧治疗脊髓损伤合并不完全性截瘫有助于患者脊髓功能恢复,可显著改善患者的运动能力及生活自理能力。

Clinical efficacy of PNF combined with hyperbaric oxygenation in the treatment of patients with spinal cord injury complicated with incomplete paraplegia

Abstract:
[Abstract] Objective To observe the clinical efficacy of proprioceptive neuromuscular facilitation(PNF) combined with hyperbaric oxygenation in the treatment of spinal cord injury complicated with incomplete paraplegia.Methods A total of 48 patients with spinal cord injury complicated with incomplete paraplegia were randomly divided into group A and group B, with 24 cases in each group. All the patients were treated with decompression of spinal cord and internal fixation of spine. All the patients started to receive rehabilitation care 1 to 2 weeks after the surgery. Group A used PNF combined with hyperbaric oxygenation therapy, while group B used routine exercise therapy and acupuncture. All the patients received rehabilitation care for 6 months. The sensation and motion before and after the treatment were evaluated according to the scaling criteria of Chinese Spinal Cord Injury Association, and the activities of daily living were evaluated according to Modified Barthel Index(MBI).Results The sensation and motion scores and MBI indices were more significantly elevated in both of the two groups after the treatment than those before the treatment(P<0.05). The increasing extents of sensation and motion scores and MBI indices were significantly greater in group A after the treatment than those in group B(P<0.05).Conclusion PNF combined with hyperbaric oxygenation in the treatment of spinal cord injury complicated by incomplete paraplegia is helpful for the recovery of spinal cord function, and significantly improves the motor ability and self-care ability.
