| ［摘要］ 目的 探讨70°鼻内镜上颌窦真菌球手术前后鼻腔鼻窦共鸣特性的改变及鼻腔阻力的变化特点。方法 对32例上颌窦真菌球患者手术前后进行主观症状视觉模拟量表评分、四相鼻阻力检查、元音共振峰检测。结果 32例患者中治愈28例，好转2例，复发2例。术后3个月患者VAS评分显著降低(P<0.01)，手术前后鼻腔通气功能无明显变化。元音i第一共振峰频率术后略有上升(P<0.05)，第二共振峰、第三共振峰及元音a的第一共振峰、第二共振峰、第三共振峰手术前后均无差别。结论 70°鼻内镜下上颌窦真菌球手术能够改善患者症状，但对鼻腔阻力无影响，可对鼻腔鼻窦共鸣特性发生轻微变化，对某些从事语音声乐工作的患者应提前告知。 |

| ［Abstract］ Objective To explore the changes of resonance characteristics of the nasal cavity, including nasal sinuses and nasal airway resistance(NAR)after transnasal 70°endoscopic surgery of fungus ball maxillary sinusitis.Methods Visual analog scale, NAR and acoustic analyses were performed on 32 patients with fungus ball maxillary sinusitis before and after the surgery.Results Of the 32 patients, 28 were cured, 2 improved and 2 relapsed.The scores of VSA scale were significantly reduced three months after the surgery(P<0.01). The effective resistances in inspiration, expiration and vertex resistance in inspiration,expiration were not significantly different before and after the surgery. There was an increase in the values of the vowel in the first formant. No significant differences were found before and after the surgery regarding fundamental frequency, the first, second and third formants of the vowel.Conclusion The transnasal 70°endoscopic surgery can improve the symptoms of the patient with fungus ball maxillary sinusitis. The surgery has no influence on the resistance of the nasal cavity. The resonance characteristics of the nasalcavity are changed slightly after the surgery. Some patients such as voice professionals should be informed about the possible alterations in speech before the surgery. |
