E-cervix宫颈弹性成像辅助诊断非孕期宫颈机能不全的研究

摘要:
[摘要] 目的 探讨E-cervix宫颈弹性成像辅助诊断非孕期宫颈机能不全的价值。方法 纳入2018-02~2018-05于该院超声科检查的非孕期宫颈机能不全患者46例作为病例组,纳入同期检查的正常的非孕期经产妇73名作为对照组。采用经阴道超声检查的方式,以宫颈正中矢状切面为初始切面,获取3幅连续存储的弹性图像。启动E-cervix技术软件包,自动测量弹性对比指数(ECI)、硬度比值(HR)、宫颈内、外口应变值(IOS、EOS)、宫颈内外口应变比值(IOS/EOS)、宫颈长度(CL),比较两组间各参数的差异,并绘制受试者工作特征(ROC)曲线。结果 病例组患者的ECI高于对照组(P<0.05),HR小于对照组(P<0.001),IOS、EOS均高于对照组(P<0.001);病例组CL短于对照组(P<0.05)。ROC曲线结果显示,HR可作为诊断非孕期宫颈机能不全较敏感的指标。结论 与正常妇女相比,宫颈机能不全患者的宫颈相对偏软且不均质。E-cervix技术可作为非孕期诊断宫颈机能不全的辅助手段。

Abstract:
[Abstract] Objective To explore the value of Elastoscan for cervix(E-cervix) technique in the auxiliary diagnosis of cervical insufficiency in non-pregnant women. Methods Forty-six cases of non-pregnant women with cervical insufficiency in our hospital from February 2018 to May 2018 were enrolled in the case group, while 73 normal non-pregnant women were enrolled in the control group during the same period. Three consecutive elastic images were obtained by transvaginal ultrasonography with the sagittal section of the cervix as the initial section. Elastic contrast index(ECI), hardness ratio(HR), internal and external ostium(IOS, EOS), IOS/EOS, and cervical length(CL) were automatically measured after the E-cervix software package was started, and the above parameters were compared between the two groups. The Receiver Operating Characteristic(ROC) Curve of the subjects was drawn to evaluate the diagnostic efficiency. Results ECI of the case group was higher than that of the control group(P<0.05); HR of the case group was less than that of the control group(P<0.001); IOS and EOS of the case group were greater than those of the control group(P<0.001); CL of the case group was shorter than that of the control group(P<0.05). The results of ROC curve showed that HR was a sensitive single index for the diagnosis of cervical insufficiency in the non-pregnant women. Conclusion Compared with normal women, the patients with cervical insufficiency have relatively soft and heterogeneous cervixes. E-cervix technique can be used as an important index for assistant diagnosis of cervical insufficiency in non-pregnant women.
