颈动脉支架置入术后再生斑块稳定性的超声造影评估

摘要:
[摘要] 目的 评估颈动脉支架置入术后再生斑块稳定性的超声造影情况。方法 选取该院2016-01~2019-08收治的40例颈动脉支架置入术后再生斑块患者为研究对象,二维超声检查出65个再生斑块。根据颈动脉粥样硬化斑块二维超声特征分为软斑组(n=29)、硬斑组(n=21)和混合斑组(n=15),观察斑块(超声造影)显影情况,并进行定量分析。结果 软斑组新生血管分级高于硬斑组和混合斑组(P<0.05),硬斑组和混合斑组新生血管分级差异无统计学意义(P>0.05);软斑组增强率均高于硬斑组和混合斑组,混合斑组增强率高于硬斑组(P均<0.05);软斑组峰值强度、达峰时间及平均渡越时间均显著低于硬斑组和混合斑组(P均<0.05),硬斑组和混合斑组峰值强度、达峰时间及平均渡越时间比较差异无统计学意义(P>0.05)。结论 超声造影能显示颈动脉斑块新生血管,判断颈动脉支架置入术后再生斑块稳定性,对防止脑血栓形成具有重要意义,有较好的实用价值。

Abstract:
[Abstract] Objective To estimate the stability of regenerated plaques after carotid stent implantation by contrast-enhanced ultrasound. Methods Forty patients with regenerated plaques after carotid stent implantation in our hospital from January 2016 to August 2019 were selected as the research subjects. 65 regenerative plaques were detected by two-dimensional ultrasonography, and they were divided into soft plaque group(n=29), hard plaque group(n=21) and mixed plaque group(n=15) according to the two-dimensional ultrasound features of carotid atherosclerotic plaques. The development of plaques(contrast-enhanced ultrasound) was observed and the results were analyzed quantitatively. Results The classification of neovascularization in the soft plaque group was significantly higher than that in the hard plaque group and the mixed plaque group(P<0.05). There was no significant difference in the classification of neovascularization between the hard plaque group and the mixed plaque group(P>0.05). The enhancement rate in the soft plaque group was significantly higher than that in the hard plaque group and the mixed plaque group, and the enhancement rate of the mixed plaque group was significantly higher than that of the hard plaque group(P<0.05). The peak strength, the peak time and the average transit time in the soft plaque group were significantly lower than those in the hard plaque group and the mixed plaque group(P<0.05). There were no significant differences in the peak strength, the peak time and the average transit time between the hard plaque group and the mixed plaque group(P>0.05). Conclusion Contrast-enhanced ultrasound can show the neovascularization of carotid plaques, and evaluate the stability of regenerated plaques after carotid stent implantation, which is of great significance and practical value to prevent cerebral thrombosis.
