VI-RADS评分对膀胱癌肌层浸润的诊断价值研究

摘要:
[摘要] 目的 探讨膀胱影像报告和数据系统(VI-RADS)评分对膀胱癌肌层浸润的诊断价值。方法 选择2018年5月至2022年7月安徽医科大学附属滁州医院收治的膀胱癌患者59例(共60个病灶),均为尿路上皮癌。患者均接受多参数磁共振(mp-MRI)检查,由2位影像科医师独立对患者的检查图像按照VI-RADS评分标准进行评分,并与病理结果进行比对。采用受试者工作特征(ROC)曲线分析VI-RADS评分对膀胱癌肌层浸润的诊断效能。结果 60个病灶中,非肌层侵犯性膀胱癌(NMIBC)38个,肌层侵犯性膀胱癌(MIBC)22个;VI-RADS评分为1分6例,2分21例,3分17例,4分12例,5分4例。2位影像科医师的VI-RADS评分结果一致性良好(Kappa=0.755,P<0.001)。ROC曲线分析结果显示,VI-RADS评分可有效诊断膀胱癌肌层侵犯情况[AUC(95%CI)=0.880(0.790~0.971),P<0.001];最佳截断值为3.5分,其对应的灵敏度为63.64%,特异度为94.74%。结论 VI-RADS评分对膀胱癌肌层浸润有较好的诊断效能,值得在临床推广。

A study on the diagnostic value of VI-RADS score for muscle invasive bladder cancer

Abstract:
[Abstract] Objective To investigate the diagnostic value of Vesical Imaging-Reporting and Data System(VI-RADS) score for muscle invasive bladder cancer. Methods Fifty-nine bladder cancer patients(60 lesions) admitted to Chuzhou Hospital Affiliated to Anhui Medical University from May 2018 to July 2022 were selected, all of which were urothelial carcinoma. All the patients underwent multiparameter magnetic resonance imaging(mp-MRI) examination. Two radiologists independently scored the examined images of the patients according to the VI-RADS scoring standard, and compared the imaging results with the pathological results. The receiver operating characteristic(ROC) curve was used to analyze the diagnostic efficacy of VI-RADS score for muscle invasive bladder cancer. Results Among the 60 lesions, there were 38 cases of non-muscle invasive bladder cancer(NMIBC) and 22 cases of muscle invasive bladder cancer(MIBC). The VI-RADS scores were 1 point in 6 cases, 2 points in 21 cases, 3 points in 17 cases, 4 points in 12 cases, and 5 points in 4 cases. The VI-RADS score results from the two radiologists were in good agreement(Kappa=0.755, P<0.001). The results of ROC curve analysis showed that VI-RADS score could effectively diagnose muscle invasive bladder cancer[area under the curve(AUC)(95%CI)=0.880(0.790-0.971), P<0.001]. The optimal cut-off value was 3.5, with a sensitivity of 63.64% and a specificity of 94.74%. Conclusion VI-RADS score has good diagnostic efficacy for muscle invasive bladder cancer, which is worthy of clinical promotion.
