冷凝集对血细胞分析多项参数检测结果的影响

摘要:
[摘要] 目的 探讨红细胞冷凝集对血细胞分析各项检测参数的影响及处理方法。方法 选择福建省肿瘤医院2017年12月至2020年2月有红细胞冷凝集的全血标本73份。应用Sysmex XN-9000全自动血细胞分析仪对标本进行检测,比较37℃水浴前后红细胞(RBC)、血细胞比容(HCT)、平均红细胞体积(MCV)、平均红细胞血红蛋白含量(MCH)、平均红细胞血红蛋白浓度(MCHC)、白细胞(WBC)、血红蛋白(Hb)和血小板(PLT)的结果差异。结果 RBC、HCT、MCV、MCH、MCHC在经水浴处理前后其结果差异有统计学意义(P<0.05),而WBC、Hb和PLT结果差异无统计学意义(P>0.05)。结论 冷凝集标本会使RBC、HCT、MCV、MCH、MCHC指标结果失真,WBC、Hb和PLT指标结果不受冷凝集干扰。在血细胞分析时,检验人员应加强质量控制,及时发现冷凝集标本并通过37 ℃水浴处理,排除干扰。

Abstract:
[Abstract] Objective To investigate the effect of erythrocyte cold agglutination on various detection parameters of blood cell analysis and the treatment methods. Methods Seventy-three whole blood specimens with erythrocyte cold agglutination from December 2017 to February 2020 in Fujian Cancer Hospital were selected. The Sysmex XN-9000 automatic blood cell analyzer was used to detect the samples, and the differences in the result parameters of red blood cell(RBC), hematocrit(HCT), mean corpuscular volume(MCV), mean corpuscular hemoglobin(MCH), mean corpuscular hemoglobin concentration(MCHC), white blood cell(WBC), hemoglobin(Hb) and platelet(PLT) were compared before and after water bath at 37 ℃. Results There were significant differences in the results of RBC, HCT, MCV, MCH and MCHC before and after water bath treatment(P<0.05), but there were no significant differences in the results of WBC, Hb and PLT(P>0.05). Conclusion The results of RBC, HCT, MCV, MCH and MCHC indexes are distorted in the cold agglutination of specimens, while the results of WBC, Hb and PLT indexes are not affected by cold agglutination. During blood cell analysis, the inspectors should strengthen quality control, find the cold agglutinated specimens in time and treat them with water bath at 37℃ to eliminate interference.
