| [摘要] 目的 探讨中耳胆脂瘤上皮中肿瘤坏死因子-α（TNF-α）在中耳胆脂瘤发生发展中的作用，研究TNF-α表达水平和患者的听力损失之间的相关性。方法 收集2017年1月-2020年1月期间新疆人民医院耳鼻喉科收治的89例中耳炎患者，其中中耳胆脂瘤患者68例（观察组），慢性化脓性中耳炎活动期患者21例（对照组），所有患者术前行纯音听阈评估听力损失。用ELISA法检测18例中耳胆脂瘤组患者（观察组）胆脂瘤上皮和6例慢性化脓性中耳炎活动期组患者（对照组）的中耳病变粘膜组织中TNF-α的表达水平，并分析其与听力损失的相关性。结果 观察组和对照组患者的TNF-α表达水平之间具有显著性差异(P＜0.05);胆脂瘤上皮中TNF-α的表达水平同听力损失各指标之间呈正相关关系（P＜0.05）。结论 中耳胆脂瘤上皮中TNF-α高表达与患者的听力损失呈正相关，即TNF-α浓度越高，患者的听力损失越严重。 |

| [Abstract] Objective In order to investigate the role of tumor necrosis factor-α (TNF-α) of cholesteatoma epithelium in the development of middle ear cholesteatoma, and to reveal the correlation between TNF-α expression and the hearing loss in these patients. Methods A total of 89 patients with otitis media treated in the Department of Otorhinolaryngology of Xinjiang People's Hospital from January 2017 to January 2020 were collected, including 68 patients with middle ear cholesteatoma (observation group) and 21 patients with the active period of chronic otitis media (control group). Before surgery, all patients were assessed by pure-tone audiometry. ELISA was used to detect TNF-α concentration in the cholesteatoma epithelium of 18 patients with middle ear cholesteatoma group (observation group) and 6 patients with the active period of chronic suppurative otitis media (control group). Then we analyzed the correlation between the expression level of TNF-α and the hearing loss. Results The results showed that there was a significant difference between the expression levels of TNF-α in patients of observation group and control group (P<0.05); and the expression level of TNF-α in cholesteatoma epithelium showed a positive correlation with 3 indexes of pure-tone audiometry (P<0.05). Conclusion The authors conclude that the high expression of TNF-α in the middle ear cholesteatoma was positively related to the hearing loss of patients, that was, the higher the TNF-α concentration was, the more severe the hearing loss of the patients was. |
