广西“小汤山”108例住院患者新型冠状病毒肺炎核酸检测结果分析

摘要:
目的分析108例住院患者新型冠状病毒肺炎(novel coronavirus pneumonia,NCP)核酸检测结果,为新型冠状病毒肺炎实验室诊断和治疗效果监测提供依据。方法收集2020年2月16日至2020年4月16日在广西壮族自治区人民医院邕武院区住院的108例新型冠状病毒肺炎确诊病例和疑似病例,采用荧光定量逆转录聚合酶链反应(real-time fluorescence reverse transcriptase polymerase chain reaction,RT-PCR) 方法检测患者呼吸道标本(咽拭子、气管吸物、痰液)和非呼吸道标本(粪便、肛拭子、结膜拭子)的新型冠状病毒核酸,分析不同临床分型NCP患者年龄性别特征和治疗后复查核酸结果,并进一步分析重型/危重型患者及婴幼儿患者治疗后不同生物学类型样本核酸结果。结果108例住院病例中确诊患者81例,无症状感染者3例,疑似患者24例。确诊患者中轻型年龄中位数为26岁,普通型年龄中位数为39岁,重型/危重型年龄中位数为66.5岁;无症状感染者年龄中位数为2岁;疑似患者年龄中位数为43.5岁。重型/危重型组患者男女比例5:3;治疗后2周复查呼吸道样本核酸阳性率分别为轻型80%(4/5),普通型36.8%(25/68),重型/危重型87.5%(7/8),无症状感染66.6%(2/3)。重型和危重型患者新型冠状病毒核酸阳性持续时间均在20天以上,治疗后多个类型样本持续阳性患者转归较差。6例婴幼儿患者治疗后咽拭子病毒核酸检测均阴性,3例患儿出现肛拭子和大便核酸检测反复阳性。结论重型/危重型组患者年龄偏高,且男性占比例较高。重型/危重型患者不同类型样本核酸阳性持续时间长,婴幼儿患者存在无症状感染且消化道病毒核酸阳性持续时间长。

Abstract:
ObjectiveTo detective 108 cases of 2019 Novel Coronavirus(2019-nCoV) nucleic acid, and provide evidence for diagnosis and treatment for novel coronavirus pneumonia(NCP). MethodsA total of 108suspected and confirmed cases of novel coronavirus pneumonia inyongwu part of The People’s Hospital of Guangxi Zhuang Autonomous Region during February 16 to April 16, 2020. Both respiratory tract samples (throat swabs, alveolar lavage fluid and sputum) and non-respiratory tract samples (stool, anal swabs, conjunctival sac swabs) were tested for 2019-nCoVby real-time fluorescence reverse transcriptase polymerase chain reaction. The NCP patients were classified into different clinical types, and their composition and positive rate of 2019-nCoVafter two weeks" treatment were observed, including various biological samples in severe and critically ill type and infants.ResultsAmong the 108 subjects,suspected was found in 81 cases, asymptomatic infection in 3 cases, suspected in 24 cases.The median age of mild type was 26, moderate type was 39,severe and critically type was66.5, asymptomatic infection was 2 and suspected case was 43.5. Five cases were male and three cases were femalein severe and critically group. After two weeks" treatment, positive rate of 2019-nCoV in different clinical types was mild type 80%(4/5),moderate type 36.8%(25/68),severe and critically type 87.5%(7/8), asymptomatic infection 66.6%(2/3). The duration of 2019-nCoV nucleic acid positive insevere and critically ill patients was more than 20 days and multiple types ofpositive samples have poor prognostic. Six infants were tested negative for throat swab after treatment but three of them had repeated positive for anal swabs and stool nucleic acid.ConclusionThesevere and critically ill patients were significantly older and have a higher proportion of men ,and had a long duration of nucleic acid positive in multiple types of samples .Infants had asymptomatic infection and the digestive tract nucleic acid positive duration was long.
