血脂蛋白相关磷脂酶A2水平与下肢动脉闭塞性疾病严重程度的相关性

摘要:
目的 分析并评估患者外周血脂蛋白相关磷脂酶A2(Lp-PLA2)水平与下肢动脉硬化闭塞症(ASO)间的相关性。方法 选择2018年11月-2019年1月在郑州大学第二附属医院就诊的108例拟诊为ASO的患者作为研究对象,根据踝肱指数(ABI)结果将其分为非ASO组(32例)和ASO组(76例)。收集两组患者基线数据、实验室数据,并对ASO组行双下肢动脉CT扫描并成像(CTA)。使用多因素logistic回归分析影响患者发生ASO的因素。对ASO组行Fontaine分期,根据CTA结果行TASCII严重程度分级,就血浆Lp-PLA2水平与ASO组下肢闭塞严重程度之间进行相关性分析。结果 比较2两组患者年龄、性别、吸烟史有统计学差异(P<0.05);研究组血Lp-PLA2水平和同型半胱氨酸(Hcy)水平高于对照组,差异有统计学意义(P<0.05);多因素logistic回归分析示年龄和Lp-PLA2是影响发生ASO的危险因素;相关性分析表明,研究组Lp-PLA2水平与ASO严重程度呈正相关。结论 血Lp-PLA2是影响ASO发生的危险因素,且随着其血浆浓度的升高,下肢动脉硬化闭塞的程度加重,血Lp-PLA2可作为评估ASO的早期检测指标。

Abstract:
Objective To investigate the correlation between lipoprotein-associated phospholipase A2(Lp-PLA2) level in patients with arteriosclerosis obliterans(ASO), and to evaluate the clinical significance. Methods 108 cases of patients were collected from November 2018 to January 2019 in our hospital, and divided into two groups according to the result of ankle-brachial index(ABI):the control group,n=32(non- arteriosclerosis obliterans), and the study group,n=76(patients with arteriosclerosis obliterans). The baseline data and laboratory data needed to be compared between two groups,and the study group underwent the dual-source CT angiography (CTA) examination. Results The age, gender and smoking history of the two groups were statistically significant (P<0.05); Compared with control group, the level of Lp-PLA2 and Hcy of the study group were higher, the difference was statistically significant (P<0.05); Multivariate logisticLogistic regression analysis indicated that the age and Lp-PLA2 are risk factors for ASO.regression indicated that the Lp-PLA2 was a risk factor for ASO patients, and the correlation analysis showed that the Lp-PLA2 level of the study group was positively correlated with the severity of ASO. Conclusion Lp-PLA2 is a risk factor affecting the occurrence of ASO, and the increase of serum Lp-PLA2 level is related to the aggravation of ASO.
