髋、膝关节置换术后恶心呕吐的防治研究进展

摘要:
髋、膝关节置换术是治疗终末期髋、膝关节疾病的有效方式,随着髋、膝关节置换手术方式愈发成熟,舒适康复已成为医生及患者共同追求的目标。由于术后剧烈疼痛、麻醉作用、术后贫血、镇痛药物应用等因素的影响,术后恶心呕吐是髋、膝关节置换术后最不理想的副作用之一,严重影响患者主观感受,明显降低术后满意度、增加并发症发生的机率,是导致患者住院时间延长的主要原因之一。鉴于术后恶心呕吐对于髋、膝关节置换患者康复的重大影响,本文重点查阅近年来国内外关于髋、膝关节置换术后防治恶心呕吐的相关文献,总结阐述髋、膝关节置换术后恶心呕吐的发生机制、影响因素、基础预防措施,旨在探讨如何针对髋、膝关节置换术后恶心呕吐的风险进行评估以及开展行之有效的防治措施,为临床治疗及研究提供一定的参考。

Abstract:
Total hip/knee arthroplasty (THA/TKA) is an effective way to treat with end-stage hip/knee diseases. With the growing maturity of THA and TKA, comfortable rehabilitation has become a common goal for doctors and patients. Due to severe postoperative pain, anesthetic effect, postoperative anemia, analgesic drug application and other factors, postoperative nausea and vomiting (PONV) is a common complication following THA and TKA. PONV can seriously affect patients" subjective feelings, and resulting in decrease in patient satisfaction and increase in healthcare costs, which is one of the main causes of prolonged hospital stay. Therefore, the prophylaxis and ultimate management of PONV are crucial for optimal patient outcomes, this paper focuses on the domestic and foreign literature on the prevention and treatment of PONA in recent years, and summarize and expound the occurrence mechanism, influencing factors, basic preventive measures and drug prevention measures of nausea and vomiting after THA and TKA. We are aiming to explore how to assess the risk of nausea and vomiting after THA/TKA and carry out effective prevention and control measures, provide some reference for clinical treatment and research.
