| 目的 探讨球棒状管胃对食管McKeown术后吻合口瘘的影响。方法 纳入苏州大学附属第一医院2022年5月1日至2023年5月1日收治的160例食管癌患者的临床资料,根据管胃制作方式分为球棒状管胃组和细管胃组。比较球棒状管胃组和细管胃组患者一般资料和食管吻合口瘘等术后并发症的发生率,并比较2组中吻合口瘘患者的热峰、发热持续时间、白细胞峰值、白细胞高持续时间、瘘诊断时间、有无纵隔脓肿(胸内瘘表现)。结果 球棒状管胃组比细管胃组吻合口瘘发生率更低,差异有统计学意义(χ2=4.335,P=0.037)。球棒状管胃组患者瘘以后热峰、白细胞峰值、白细胞高持续时间、纵隔脓肿(胸内瘘表现)发生率均明显高于细管胃组,差异有统计学意义(t=3.147,P=0.004；t=2.227,P=0.034；t=2.644,P=0.013；P=0.027)。结论 在食管McKeown术中制作球棒状管胃有助于减少吻合口瘘发生率,但它会增加胸内瘘的发生率。 |

| The Effect of Baseball Bat-like Gastric Tube on Anastomotic Leakage after McKeown Esophagectomy |

| Objective To investigate the effect of baseball bat-like gastric tube on anastomotic leakage after McKeown esophagectomy. Methods The clinical data of 160 patients with esophageal cancer admitted to the First Affiliated Hospital of Soochow University from May 1, 2022 to May 1, 2023 were included in this study. These patients were divided into two groups according to the way of gastric-tube fabrication, the baseball bat-like gastric tube group and the typical gastric tube group. The general data and the incidence of postoperative complications such as esophageal anastomosis leakage between the two groups were recorded for analysis. In patients who were diagnosed with anastomosis leakage, the fever peak, duration of feaver, leukocytes peak, duration of leukocyte hypercapnia, the time of leakage diagnosis, and the presence of mediastinal abscess (manifestation of intrathoracic anastomosis leakage) were also recorded for comparison. Results The incidence of anastomosis leakage was lower in the baseball bat-like gastric tube group than in the typical gastric tube group, and the difference was statistically significant (χ2=4.335,P=0.037). In patients who were diagnosed with anastomosis leakage, the fever peak, leukocyte peak, leukocyte high duration, and incidence of mediastinal abscess (manifestation of intrathoracic anastomosis leakage) was significantly higher in patients in the baseball bat-like gastric tube group than in the typical gastric tube group, and the difference was statistically significant (t=3.147,P=0.004；t=2.227,P=0.034；t=2.644,P=0.013；P=0.027). Conclusion Making a baseball bat-like gastric tube during McKeown esophagectomy procedure helps to reduce the incidence of anastomosis leakage, but it increases the incidence of intrathoracic anastomosis leakage. |
