[摘要] 目的 观察目标-活动-运动集成(GAME)疗法在学龄前期痉挛型偏瘫脑瘫患儿上肢功能康复中的应用效果。方法 招募2023年8月至2024年7月首都医科大学附属北京儿童医院收治的学龄前期痉挛型偏瘫脑瘫患儿56例,采用随机数字表法将其分为观察组和对照组,各28例。对照组有2例因病终止训练,1例转至外地继续康复;观察组有3例转至外地继续康复。最终观察组和对照组各有25例纳入研究。两组患儿均通过医院康复与家庭训练相结合的方式进行康复治疗。对照组接受强制性诱导运动疗法(CIMT)。观察组根据GAME疗法原则设计并开展训练。康复治疗持续3个月。比较两组治疗前后Carroll手功能评定量表(UEFT)评分和儿童生活功能评估量表(PEDI)标准分。结果 治疗前,两组UEFT评分和PEDI标准分比较差异无统计学意义(P>0.05)。治疗后,两组UEFT评分和PEDI标准分均较治疗前提高,且观察组评分高于对照组,差异有统计学意义(P<0.05)。结论 GAME疗法应用于学龄前期痉挛型偏瘫脑瘫患儿的上肢功能训练,可以更好地改善其上肢功能,提高自理能力。 |

[Abstract] Objective To observe the application effect of Goals-Activity-Motor Enrichment(GAME) therapy in upper limb functional rehabilitation for cerebral palsy pediatric patients with spastic hemiplegia at preschool age. Methods A total of 56 cerebral palsy pediatric patients with spastic hemiplegia at preschool age who were admitted to Beijing Children′s Hospital, Capital Medical University from August 2023 to July 2024 were recruited. The pediatric patients were divided into observation group and control group by random number table method, with 28 cases in each group. In the control group, the training was stopped in 2 patients due to their illness, and 1 patient was transferred to another place outside Beijing for continuous rehabilitation. In the observation group, 3 patients were transferred to other places outside Beijing for continuous rehabilitation. Finally, 25 cases in the observation group and 25 cases in the control group were included in this study. The pediatric patients in both groups received rehabilitation treatment through the combination of hospital rehabilitation and family training. The control group received constraint-induced movement therapy(CIMT). According to the principle of GAME therapy, the training program of the observation group was designed and the training was carried out. The rehabilitation treatment lasted for 3 months. Carroll Upper Extremity Function Test(UEFT) scores and Pediatric Evaluation of Disability Inventory(PEDI) standard scores were compared between the two groups before and after treatment. Results Before treatment, the UEFT scores and PEDI standard scores were not significantly different between the two groups(P>0.05). After treatment, the UEFT scores and PEDI standard scores in both groups were elevated compared with those before treatment, and the UEFT scores and PEDI standard scores in the observation group were higher than those in the control group after treatment, with statistically significant differences between the two groups(P<0.05). Conclusion The application of GAME therapy in upper limb functional training for cerebral palsy pediatric patients with spastic hemiplegia at preschool age can better improve the upper limb function and improve their self-care ability. |
