全血与去白细胞悬浮红细胞在储存过程中NETs的形成及机制探讨

摘要:
目的 探讨全血与去白细胞悬浮红细胞在储存过程中中性粒细胞胞外诱捕网(NETs)的形成及机制。方法 收集2023年01月至2024年05月广西壮族自治区人民医院输血科的库存血样,包括全血组(80例,我院储存式自体血未经去白处理)与去白细胞悬浮红细胞组(80例,来自南宁中心血站供血者)。每组根据储存时间段分为A组(1~9 d)、B组(10~19 d)、C组(20~28 d)、D组(>28 d),每组20例。通过酶联免疫吸附试验(ELISA)检测血样上清液中TNF-α、IL-8、IL-6、血红素(Heme)和髓过氧化物酶(MPO)水平,使用PicoGreen荧光分光光度法检测血样上清液中游离DNA(cf-DNA)含量。结果 去白细胞红细胞组TNF-α、IL-6、Heme、cf-DNA,MPO水平显著低于全血组(P<0.05)。全血组组TNF-α、IL-8水平随着保存时间延长逐渐上升。cf-DNA、Heme水平前期随着保存时间延长逐渐上升,在储存20~28 d时达到峰值,随后回落。去白细胞悬浮红细胞组TNF-α、Heme水平随着保存时间延长逐渐上升,cf-DNA水平随着保存时间延长逐渐下降。两组IL-6、MPO水平随着保存时间延长无明显变化。cf-DNA水平与heme,TNF-α、IL-6水平呈正相关(r=0.602,r=0.515,r=0.506),与IL-8水平呈负相关(r=-0.291);MPO与Heme、TNF-α、IL-6水平呈正相关(r=0.409,r=0.856,r=0.813),与IL-8水平无关联。结论 NETs的水平受储存时间和红细胞制备方式的影响,全血中NETs标志物水平显著高于去白细胞悬浮红细胞。

Discussion on the Formation and Mechanisms of Neutrophil Extracellular Traps in Whole Blood and Leukoreduced Red Blood Cell Suspensions during Storage

Abstract:
[Abstract]Objective Investigating the formation and mechanisms of neutrophil extracellular traps (NETs) in whole blood and leukoreduced red blood cell suspensions during storage. Methods Blood samples were collected from the Blood Transfusion Department of Guangxi Zhuang Autonomous Region People's Hospital between January 2023 and May 2024, including whole blood (80 cases; autologous stored blood from our hospital, not leukoreduced) and leukoreduced red blood cell suspensions (80 cases; donated by donors from the Nanning Central Blood Station). The study population was stratified into four groups based on red blood cell storage duration: Group A (1–9 days), Group B (10–19 days), Group C (20–28 days), and Group D (>28 days), with each group comprising 20 subjects. Serum concentrations of tumor necrosis factor-alpha (TNF-α), interleukin-8 (IL-8), interleukin-6 (IL-6), heme, and myeloperoxidase (MPO) were quantified using enzyme-linked immunosorbent assay (ELISA). Circulating cell-free DNA (cfDNA) levels in serum were determined via PicoGreen-based fluorescent spectrophotometry. Results TNF-α and IL-6 concentrations, as well as levels of heme, cf-DNA, and MPO, were significantly lower in the leukoreduced red blood cell group compared with the whole blood group (P<0.05). In contrast, in the whole blood group, TNF-α and IL-8 concentrations exhibited a progressive increase over the course of storage. cf-DNA and heme rose then fell, peaking at days 20–28. In leukoreduced red blood cell suspensions, TNF-α and heme increased steadily with storage, while cf-DNA decreased. IL-6 and MPO levels remained stable during extended storage. cf-DNA correlated positively with heme (r=0.602), TNF-α (r=0.515), and IL-6 (r=0.506), but negatively with IL-8 (r=?0.291). MPO correlated strongly with heme (r=0.409), TNF-α (r=0.856), and IL-6 (r=0.813), and showed no link to IL-8.Conclusion The levels of NETs are influenced by both the duration of storage and the method of red blood cell preparation.Specifically,the concentrations of NETs in whole blood are markedly higher than those observed in leukoreduced red blood cell suspensions.
