万古霉素联合鼻环引流辅助胫骨横向骨搬移治疗足踝部骨髓炎的疗效及对炎症反应的影响

摘要:
目的:探讨万古霉素联合鼻环引流辅助胫骨横向骨搬移治疗足踝部骨髓炎的疗效及对炎症反应的影响。方法:2023年1月~2024年12月,将邯郸市第一医院诊治的148例足踝部骨髓炎患者按随机数字表法分为:传统组、鼻环组,各74例。传统组采用万古霉素联合传统骨搬移治疗,鼻环组采用万古霉素联合鼻环引流辅助胫骨横向骨搬移治疗。对比两组疗效、炎症反应、疼痛程度及其他相关指标。结果:鼻环组骨愈合、功能优良率显著高于传统组(P<0.05)。两组术后WBC、CRP、IL-6均显著低于术前且鼻环组术后显著低于传统组(P<0.05)。两组术后视觉模拟疼痛评分(VAS)比术前显著升高且鼻环组术后显著高于传统组(P<0.05)。鼻环组抗生素使用及创面愈合时间显著低于传统组,耐甲氧西林金黄色葡萄球菌(MRSA)总清除率显著高于传统组(P<0.05)。结论:万古霉素联合鼻环引流辅助胫骨横向骨搬移相比万古霉素联合传统骨搬移治疗足踝部骨髓炎,在骨愈合、功能优良率、炎症指标控制、抗生素使用及创面愈合时间、MRSA总清除率等方面优于传统治疗,虽术后疼痛评分更高,但整体疗效好且能有效改善炎症反应。

The efficacy of vancomycin combined with nasal ring drainage-assisted tibial transverse bone transport in treating foot and ankle osteomyelitis and its impact on inflammatory response

Abstract:
Objective: To discuss the efficacy of vancomycin combined with nasal ring drainage-assisted tibial transverse bone transport in treating foot and ankle osteomyelitis, and its impact on inflammatory response. Methods: From January 2023 to December 2024, 148 patients with ankle osteomyelitis treated in Handan First Hospital were randomly separated into a traditional group and a nasal ring group, each with 74 cases. The traditional group adopted vancomycin combined with traditional bone transport, while the nasal ring group adopted vancomycin combined with nasal ring drainage-assisted tibial transverse bone transport. The efficacy, inflammatory response, pain level, and other related indicators were compared. Results: The nasal ring group had clearly higher rates of bone healing and functional excellence than the traditional group (P<0.05). The postoperative WBC, CRP, and IL-6 in both groups were prominently lower than before surgery, and the nasal ring group was prominently lower than the traditional group after surgery (P<0.05). The postoperative visual analogue pain scores (VAS) of both groups were prominently higher than before surgery, and the nasal ring group was clearly higher than the traditional group after surgery (P<0.05). The nasal ring group had clearly lower antibiotic use time and wound healing time than the traditional group, and a prominently higher total clearance rate of methicillin-resistant Staphylococcus aureus (MRSA) than the traditional group (P<0.05). Conclusion: Vancomycin combined with nasal ring drainage-assisted tibial transverse bone transport is superior to vancomycin combined with traditional bone transport in treating foot and ankle osteomyelitis in terms of bone healing, functional excellence rate, inflammation index control, antibiotic use time, wound healing time, and total MRSA clearance rate. Although it has a higher postoperative pain score, it has good overall efficacy and can effectively improve inflammatory response.
