摘要:
本指南基于国际功能、残疾和健康分类（International Classification of Functioning, Disability and Health, ICF）的理论框架，结合当前最佳循证医学证据，系统构建了心脏康复（Cardiac Rehabilitation, CR）的评估、干预与管理方案。内容涵盖ICF核心分类组合在心脏康复中的应用、功能评估工具选择与标准、危险分层标准、个体化干预与管理等关键环节，旨在为临床工作者提供标准化、系统化的实践指导，提升心脏康复的服务质量与患者功能结局。

Abstract:
This guideline is developed based on the theoretical framework of the International Classification of Functioning, Disability and Health, combined with the current best evidence-based medical evidence, to systematically construct the assessment, intervention and management protocols of cardiac rehabilitation. It covers key components including the application of ICF core sets in cardiac rehabilitation, the selection and criteria of functional assessment tools, risk stratification criteria, as well as individualized intervention and management. This guideline aims to provide clinicians with standardized and systematic practical guidance, so as to improve the quality of cardiac rehabilitation services and patients' functional outcomes.
