青春期极重型再生障碍性贫血患者生育力保存一例并文献复习

摘要:
[摘要] 目的 回顾1例青春期极重型再生障碍性贫血(VSAA)患者的诊治经过,探讨青春期女性通过卵巢组织冷冻(OTC)联合未成熟卵母细胞体外成熟(IVM)技术行生育力保存的效果。方法 2025年7月,广西壮族自治区人民医院收治1例青春期拟行造血干细胞移植(HSCT)的VSAA患者,通过OTC联合IVM技术行生育力保存,收集其诊疗资料。检索国内外生育力保存的文献报道,分析患者的诊断及生育力保存情况。结果 患者卵巢组织获取术后,体外穿刺抽吸共获取3枚卵丘-卵母细胞复合体(COCs)及10片卵巢组织皮质片。COCs经48 h IVM后共有2枚MⅡ期卵母细胞行玻璃化冷冻。结论 对亟待HSCT的青春期女性患者,OTC联合IVM技术可能在短期内最大程度地保存VSAA患者的生育力。

Fertility preservation in an adolescent patient with very severe aplastic anaemia and literature review

Abstract:
[Abstract] Objective To review the diagnosis and treatment process of an adolescent patient with very severe aplastic anaemia(VSAA), and to explore the effectiveness of fertility preservation for adolescent females through ovarian tissue cryopreservation(OTC) combined with in vitro maturation(IVM) of immature oocytes technique. Methods An adolescent VSAA patient was admitted to the People′s Hospital of Guangxi Zhuang Autonomous Region and was scheduled for hematopoietic stem cell transplantation(HSCT) in July 2025. Before HSCT, the patient underwent fertility preservation through OTC combined with IVM technique, and her diagnosis and treatment data were collected. By searching domestic and overseas literature reports on fertility preservation, the diagnosis and fertility preservation of the patient were analyzed. Results After ovarian tissue extraction, a total of 3 cumulus-oocyte complexes(COCs) and 10 pieces of ovarian cortex tissues of the patient were obtained through puncture in vitro and aspiration. After IVM for 48 h, a total of 2 oocytes at the metaphase Ⅱ(MⅡ) stage in COCs were freezed by using vitrification. Conclusion For the adolescent female patients in urgent need of HSCT for treating VSAA, OTC in combination with IVM technique may preserve their fertility to the greatest extent in the short term.
