维持性血液透析患者支持性照顾需求的现状及影响因素分析

[摘要] 目的 分析维持性血液透析(MHD)患者支持性照顾需求的现状及影响因素,为临床早期识别高需求人群,制订个体化干预措施提供依据。方法 采用便利抽样法,选取武汉科技大学附属武昌医院2025年3月至7月200例门诊MHD患者作为研究对象。采用一般资料调查问卷、支持性照顾需求量表(SCNS-SF34)、终末期肾病维持性血液透析患者治疗依从性量表、汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)对患者进行问卷调查。采用Pearson相关分析探讨MHD患者支持性照顾需求、焦虑抑郁和治疗依从性之间的相关性。采用多重线性回归分析评估MHD患者支持性照顾需求的影响因素。结果 200例MHD患者SCNS-SF34总分为(78.98±14.86)分。不同年龄、教育水平、工作情况、平均月收入的MHD患者SCNS-SF34总分比较差异有统计学意义(P<0.05)。MHD患者SCNS-SF34总分与HAMA评分呈正相关(r=0.774,P<0.001),与HAMD评分呈正相关(r=0.322,P<0.001);治疗依从性量表评分与HAMA评分呈负相关(r=-0.346,P<0.001),与HAMD评分呈负相关(r=-0.224,P=0.001);SCNS-SF34总分与治疗依从性量表评分呈负相关(r=-0.446,P<0.001)。多重线性回归分析结果显示,年龄、教育水平、工作情况、HAMA评分、治疗依从性量表评分是MHD患者支持性照顾需求的影响因素(P<0.05)。结论 MHD患者存在未满足的支持性照顾需求,且受年龄、教育水平、工作情况、焦虑程度、治疗依从性的影响。

Analysis on the current status and influencing factors of supportive care needs in patients receiving maintenance hemodialysis

Abstract:
[Abstract] Objective To analyze the current status and influencing factors of supportive care needs in patients receiving maintenance hemodialysis(MHD), and to provide a basis for the early clinical identification of the populations with high supportive care needs and the formulation of individualized intervention measures. Methods Two hundred outpatients receiving MHD in Wuchang Hospital Affiliated to Wuhan University of Science and Technology from March 2025 to July 2025 were selected as the research subjects by using convenience sampling method. Questionnaire surveyrs were conducted on the patients by using a general data questionnaire, 34-item Supportive Care Needs Surveys(SCNS-SF34), End-Stage Renal Disease Maintenance Hemodialysis Patient Treatment Adherence Scale, Hamilton Rating Scale for Anxiety(HAMA), and Hamilton Rating Scale for Depression(HAMD). Pearson correlation analysis was used to explore the correlations of supportive care needs, anxiety and depression with treatment compliance in patients receiving MHD. Multiple linear regression analysis was used to assess the factors influencing supportive care needs in patients receiving MHD. Results The total scores of SCNS-SF34 in the 200 patients receiving MHD were (78.98±14.86)points. There were statistically significant differences in SCNS-SF34 total scores among the MHD patients with different ages, educational levels, jobs and average monthly earnings(P<0.05). The SCNS-SF34 total scores in patients receiving MHD were positively correlated with HAMA scores(r=0.774, P<0.001) and HAMD scores(r=0.322, P<0.001). The scores of the treatment compliance scale in the patients were negatively correlated with HAMA scores(r=-0.346, P<0.001) and HAMD scores(r=-0.224, P=0.001). The SCNS-SF34 total scores were negatively correlated with the scores of the treatment compliance scale(r=-0.446, P<0.001). The results of multiple linear regression analysis showed that age, educational level, jobs, HAMA scores and the scores of the treatment compliance scale were the factors influencing supportive care needs in patients receiving MHD(P<0.05). Conclusion Patients receiving MHD have unmet supportive care needs, which are influenced by age, educational level, jobs, anxiety degree and treatment compliance.
