2型糖尿病患者足部电流感觉阈值异常的相关因素分析

摘要:
[摘要] 目的 分析2型糖尿病患者足部电流感觉阈值(CPT)异常的相关因素,旨在识别感觉神经功能异常的高风险人群,为临床早期预警和照护干预提供依据。方法 回顾性分析2024年12月至2025年12月在安徽中医药大学第一附属医院内分泌科住院的169例2型糖尿病患者的临床资料。根据CPT检测结果将患者分为异常组(90例)和正常组(79例)。比较两组临床资料,采用多因素logistic回归分析CPT异常的相关因素。结果 异常组年龄大于正常组,糖尿病病程长于正常组,尿蛋白/肌酐比值(UACR)高于正常组,估算肾小球滤过率(eGFR)低于正常组,差异有统计学意义(P<0.05)。多因素logistic回归分析结果显示,糖尿病病程延长[OR(95%CI)=1.050(1.002~1.092),P=0.041]、log-UACR升高[OR(95%CI)=2.150(1.380~3.347),P<0.001]、eGFR降低[OR(95%CI)=0.970(0.952~0.994),P=0.012]与CPT异常有关。结论 糖尿病病程延长、UACR升高及eGFR降低与CPT异常存在独立关联。密切监测上述关联指标,可为糖尿病周围神经病变的早期防治提供有价值的临床参考。

Analysis on the associated factors of abnormal current perception threshold in the feet of patients with type 2 diabetes mellitus

Abstract:
[Abstract] Objective To analyze the associated factors of abnormal current perception threshold(CPT) in the feet of patients with type 2 diabetes mellitus, aiming to identify high-risk individuals with abnormal sensory nerve function and provide a basis for early clinical warning and care intervention. Methods A retrospective analysis was conducted on the clinical data of 169 patients with type 2 diabetes mellitus who were admitted to Department of Endocrinology, the First Affiliated Hospital of Anhui University of Chinese Medicine from December 2024 to December 2025. According to the detecting results of CPT, the patients were divided into abnormal group(90 patients) and normal group(79 patients). The clinical data were compared between the two groups. Multivariate logistic regression was used to analyze the associated factors of abnormal CPT. Results Compared with the normal group, the abnormal group was older and had a longer course of diabetes mellitus, a higher urinary albumin-to-creatinine ratio(UACR), and a lower estimated glomerular filtration rate(eGFR), with statistically significant differences between the two groups(P<0.05). The results of multivariate logistic regression analysis showed that prolonged course of diabetes mellitus[OR(95%CI)=1.050(1.002-1.092), P=0.041], elevated log-UACR [OR(95%CI)=2.150(1.380-3.347), P<0.001] and decreased eGFR[OR(95%CI)=0.970(0.952-0.994), P=0.012] were associated with abnormal CPT. Conclusion The prolonged course of diabetes mellitus, elevated UACR and decreased eGFR are independently associated with abnormal CPT. Close monitoring of these associated indicators can provide valuable clinical reference for the early prevention and treatment of diabetic peripheral neuropathy.
