舒芬太尼在无痛胃镜术中的应用

摘要:
[摘要] 目的 探讨舒芬太尼应用于无痛胃镜术中的安全性和有效性。方法 将80例ASA评分Ⅰ~Ⅱ级,拟行无痛胃镜检查术的患者随机分为舒芬太尼组(S组)和芬太尼组(F组),每组40例。分别采用舒芬太尼0.15 μg/kg或芬太尼1.0 μg/kg复合丙泊酚2 mg/kg行静脉全麻。观察麻醉前、术中、手术结束、苏醒时、术后30 min各时点病人的血压、心率、呼吸及镇静镇痛分级,记录手术时间、苏醒时间、手术室滞留时间以及恶心呕吐等副作用,随访术后24 h腹痛及副反应。结果 术后30 min及4 h的VAS评分两组差异有统计学意义(P<0.05)。丙泊酚用量两组差异无统计学意义(P>0.05)。术中、术毕、苏醒时镇静分级差异无统计学意义(P>0.05),血压、心率及副反应方面差异无统计学意义(P>0.05)。结论 舒芬太尼0.15 μg/kg复合丙泊酚2 mg/kg可安全用于无痛胃镜术,其更长的镇痛时间在病人术后的舒适度方面较芬太尼有更大的优越性。

Abstract:
[Abstract] Objective To evaluate the effect of sufentanil anesthesia in painless gastroscopy.Methods Eighty patients were randomly divided into two groups: sufentanil group (S,n=40) and fentanyl group (F,n=40). Total intravenous anesthesia was induced either with sufentanil 0.15 μg/kg propofol 2 mg/kg or fentanyl 1 μg/kg propofol 2 mg/kg. The anesthesia effcet was evaluated. The analgesic degree, the blood pressure, heart beats,breathing was monitored before anesthesia, during operation and 30 minutes after operation respectively. The intervals of operation, consciousness recovery, staying in operation room were recorded.The side effect such as nausea and vomitting was also observed during operation and followed for 4 hours after operation.Results There was significant difference in VAS scores at 30 minutes and 4 hours after operation (P<0.05) between two groups. There were not significant difference in the dose of propofol, the anesthesic degree during and after operation, blood pressure,heart beats and side effect between two groups(P>0.05).Conclusion Sufentanil can be administered in painless gastroscopy. The intervals of anesthesia was longer and comfortable sense was better as compared with fentanyl.
