氨茶碱联合纳络酮治疗早产儿原发性呼吸暂停的疗效观察

摘要:
[摘要] 目的 观察氨茶碱联合纳络酮治疗早产儿原发性呼吸暂停的临床疗效。方法 将52例发生呼吸暂停的早产儿随机分为两组,治疗组和对照组各26例。两组均应用氨茶碱,首剂5 mg/kg,12 h后给予维持量2.5 mg/kg,q 12 h,治疗组在上述治疗基础上加用纳络酮0.1 mg/kg静脉推注,继之予0.1~0.2 mg/kg,按0.03~0.05 mg/(kg·h)速度持续静脉泵入,6~8 h/d,两种药物均用3~5 d。结果 治疗组显效19例(73.07%),有效6例(23.07%),无效1例(3.84%);对照组显效10例(38.46%),有效8例(30.76%),无效8例(30.76%)。总有效率治疗组96.15%,对照组69.23%,两组比较差异有统计学意义(χ2=4.84,P<0.05)。结论 氨茶碱联合纳络酮治疗早产儿原发性呼吸暂停疗效显著。

Abstract:
[Abstract] Objective To observe the efficiency of aminophylline combined with naloxone in treatment of premature infants with primary apnea.Methods Fifty-two patients were randomly divided into two groups:the treatment group and the control group, each group contained 26 cases. Aminophylline were used in both groups,the first dose was 5 mg/kg,after 12 hours,followed by 2.5 mg/kg,one time per 12 hours.The treatment group was added naloxone in an initial dose of 0.1 mg/kg intravenous injection on the basis of the control group, followed by naloxone 0.1~0.2 mg/kg, at a speed of 0.03~0.05 mg/(kg·h), 6~8 h every day, continuous intravenous infusion for 3~5 days.Results In the treatment group,marked effect was found in 19 cases(73.07%), improvement in 6 cases (23.07%)and no effect in 1 case(3.84%); In the control group marked effect in 10 cases (38.46%),improvement in 8 cases(30.76%),no effect in 8 cases (30.76%).The total effective rate of the treatment group was 96.15%, that of the control group was 69.23%.The difference of the total effective rates between two groups was significant (χ2=4.84,P<0.05).Conclusion Aminophylline plus naloxone in treatment of premature infant with primary apnea is effective.
