| ［摘要］ 目的 评估尿酸对扩张型心肌病（DCM）心衰患者预后的价值。方法 检测78例DCM患者入院时血尿酸（SUA），并进行中位数为13.5（3～25）个月的随访，观察心血管事件（心源性死亡及心衰失代偿再入院）情况。结果 失访4例，事件组38例，非事件组36例。(1)与非事件组相比，事件组SUA更高［(576.7±182.9)μmol/L vs (462.4±164.2)μmol/L］（P<0.01）。(2)SUA对预后判断的ROC曲线下面积为0.673，最佳截定点565 μmol/L，灵敏度0.579，特异度0.778。（3）Kaplan-meier生存曲线分析表明SUA>565 μmol/L组的再发心血管事件风险是SUA≤565 μmol/L组的4.813倍（95%CI：1.742～13.292，P=0.002）。结论 SUA水平对DCM患者预后评估有重要价值。SUA>565 μmol/L提示预后不良。 |

| ［Abstract］ Objective To evaluate the prognostic value of serum uric acid(SUA) in patients with dilated cardiomyopathy(DCM) complicated by heart failure（HF）.Methods Seventy-eight patients hopitalitalized with DCM were included in this study. SUA were measured on admission. All patients discharged were followed up for a median of 13.5 months (3～25 months).The cardiovascular event were observed including cardiac death or readmission due to decompensated HF.Results Among 78 patients, 4 patients failed to be followed up，there were 38 patients in the events group，36 patients in non-event group.(1)Compared with non-event group, the event group had a significiant higher SUA level ［(576.7±182.9)μmol/L vs (462.4±164.2)μmol/L］, P<0.01.(2)Area under the curve for SUA in evaluating prognosis of DCM patients was 0.673,optimal cut point was 565 μmol/L,sensitivity 0.579,specificity 0.778.(3)The analysis of Kaplan-Meier survival curve showeded,SUA>565 μmol/L on adminssion were associated with increased cardiovascular events.The risk of recurrent cardiovascular events of group with SUA>565 μmol/L was 4.813 times as much as that of group with SUA≤565 μmol/L(Hazard Ratio［HR］=4.813, 95%CI：1.742～13.292，P=0.002).Conclusion SUA level has important prognosis value in patients with DCM. SUA>565 μmol/L predicted an unfavourable prognosis. |
