CT灌注成像对不同程度颈内动脉狭窄患者脑组织脑灌注效果的评价

摘要:
[摘要] 目的 通过CT灌注(CTP)成像对不同程度颈内动脉狭窄患者脑组织脑灌注效果进行评价,为颈内动脉狭窄患者脑缺血的临床诊断及治疗提供依据。方法 收集2010-06~2012-06不同程度的单侧颈内动脉狭窄患者48例,观察不同程度颈内动脉狭窄患者脑组织不同部位脑灌注状态。结果 相对灌注开始时间(rTS):闭塞患者脑颞叶、基底节、前分水岭灰质、前分水岭白质、后分水岭灰质、后分水岭白质较中度狭窄患者明显延长(P<0.05);闭塞患者前分水岭灰质、前分水岭白质、后分水岭白质较重度狭窄患者明显延长(P<0.05)。相对灌注达峰时间(rTTP):闭塞患者脑颞叶、基底节、前分水岭白质、后分水岭灰质较中度狭窄患者明显延长(P<0.05);闭塞患者基底节、前分水岭灰质、前分水岭白质较重度狭窄患者明显延长(P<0.05)。结论 颈内动脉狭窄患者脑组织脑灌注状态可通过CTP进行有效评价,随着颈内动脉狭窄程度的增加,灌注异常也明显增加,而以前、后分水岭区多见,尤其是后分水岭区白质最明显。

Abstract:
[Abstract] Objective To evaluate the cerebral perfusion resulting from different degrees of stenosis carotid artery by CT perfusion(CTP) imaging and to provide the basis for clinical diagnosis and treatment of carotid artery stenosis cerebral ischemia.Methods Forty-eight patients with unilateral carotid artery stenosis from June 2010 to June 2012 were selected. The cerebral perfusion states of different parts of the brain in the patients with different degrees of stenosis carotid artery were observed.Results rCBF, rCBV of different anatomic sites of the brain tissues in patients with different narrow degrees of unilateral internal carotid artery were not significantly different. The revised trauma scores(rTS) of cerebral temporal leaf, basal ganglia, anterior watershed gray matter, anterior watershed white matter,after watershed gray matter, after watershed white matter of occlusion patients were longer than those of moderate stenosis patients(P<0.05); the rTS of anterior watershed gray matter, anterior watershed white matter, after watershed white matter of occlusion patients were longer than those of severe stenosis patients(P<0.05); the rTTP of cerebral temporal leaf, basal ganglia, anterior watershed white matter, after watershed gray matter of occlusion patients were longer than those of moderate stenosis patients(P<0.05); the rTTP of basal ganglia, anterior watershed gray matter, anterior watershed white matter of occlusion patients were longer than those of moderate stenosis patients(P<0.05).Conclusion The cerebral perfusion states of patients with carotid artery stenosis can be effectively evaluated by CTP. With the increasing degrees of the carotid stenosis, the perfusion abnormalities increase significantly, especially in anterior watershed and after watershed, and in white matter after watershed.
