| ［摘要］ 目的 对比分析股骨粗隆间骨折患者经不同方式治疗的临床效果。方法 随机抽取104例该院于2013-01～2014-12收治的股骨粗隆间骨折患者进行研究，按入院顺序半随机分为对照组和观察组各52例；对照组给予动力髋螺钉（dynamic hip screw，DHS）治疗，观察组给予股骨近端髓内钉（proximal femoral nail，PFN）治疗；对比两组患者的手术情况、术后康复效果以及Harris（髋关节功能）评分。结果 （1）两组患者的手术时间和术中出血量差异无统计学意义（P>0.05）。（2）观察组患者的骨折愈合时间明显短于对照组，差异有统计学意义（P<0.05）；但两组的并发症发生率比较，差异无统计学意义（P>0.05）。（3）治疗后，两组患者的Harris评分均明显提高，且观察组高于对照组（P<0.05），治疗前后和组间比较差异均有统计学意义（P<0.05）。结论 与DHS比较，PFN治疗股骨粗隆间骨折具有更显著的临床效果，可以缩短骨折愈合时间，促进关节功能的恢复，更适合用于临床推广。 |

| ［Abstract］ Objective To explore the clinical effects of different operations on intertrochanteric fractures.Methods One hundred and four cases with intertrochanteric fracture in our hospital from January 2013 to December 2014 were randomly divided into the control group(n=52) and the observation group(n=52). The observation group was treated with Dynamic Hip screws and the control group was given proximal femoral intramedullary nailing. The clinical effect and Harris scores were compared between the two groups.Results (1)There were no significant diflerences in the operation time and intraoperative blood loss between the two groups(P>0.05). (2)The fracture healing time of the observation group was significantly shorter than that of the control group(P<0.05). There was no statistically significant difference in the incidence of complications between the two groups(P>0.05). (3)After the treatment, Harris scores were significantly improved in both of two groups, however Harris scores in the observation group is higher than those in the control group(P<0.05).Conclusion PFN is more effective that DHS in the treatment of intertrochanteric fractures. |
