城市社区医护人员心电图知识和技能现状调查及培训效果观察

摘要:
[摘要] 目的 了解城市社区医护人员心电图知识和技能的现状及培训后效果,探讨培训对策。方法 选择城市社区医护人员83名,其中初级65名,中级18名,无高级职称。所有社区医护人员均采用自制调查表,以集中培训的方式,进行培训前后问卷调查及心电图理论授课和技能培训,了解和考核社区医护人员在培训前后对正常心电图、急性心肌梗死(AMI)心电图相关知识、心电图阅图及心电图仪操作技能掌握情况(正确率),并进行比较。结果 研究显示培训前考核:(1)心电图相关理论知识:正常心电图知晓率初级为43.1%~44.6%,中级为16.7%~66.7%;AMI心电图诊断知晓率初级为44.6%~90.8%,中级为22.2%~100.0%。(2)心电图技能操作:心电图阅图分析正确率初级为26.2%,中级为38.9%;心电图仪操作掌握熟练程度(合格率)初级为58.5%,中级为83.3%。经培训后考核结果显示:(1)心电图相关理论知识:正常心电图知晓率初级为70.8%~84.6%,中级为88.9%~94.4%;AMI心电图诊断知晓率初级为50.8%~98.5%,中级为27.8%~100.0%。(2)心电图技能操作:心电图阅图分析正确率初级为49.2%,中级为55.6%;心电图仪操作掌握熟练程度(合格率)初级提升至75.4%,中级提升至94.4%。各项内容培训前后比较差异均有统计学意义(P<0.05)。结论 社区医护人员心电图知识和技能亟待提高,深入社区开展心电图技能培训能有效地提高院前诊断AMI的水平。

Abstract:
[Abstract] Objective To investigate the awareness status of relevant knowledge about electrocardiogram(ECG) and the skill training effect on the medical workers in urban communities and to explore the countermeasures for the improvement of teaching quality.Methods Eighty-three medical workers from the urban communities with middle-level professional titles in 18 cases and primary-level titles in 65 cases. All the community medical workers were investigated with a self-designed questionnaire. The centralized training ECG theory and practice was performed in all the cases. The training results were compared before and after the training.Results Before the training:(1) the awareness rates of normal ECG and AMI ECG were 43.1%~44.6% and 44.6%~90.8% respectively in the medical workers with primary-level professional titles, and 16.7%~66.7% and 22.2%~100.0% respectively in the medical workers with middle-level professional titles;(2)the rates of proper analysis of ECG and proper operation of ECG were 26.2% and 58.5% in the medical workers with primary-level professional titles, and 38.9% and 83.3% respectively in the medical workers with middle-level professional titles. After the training:(1)the awareness rates of normal ECG were 70.8%~84.6% and 88.9%~94.4% in the medical workers with primary-level and in those with middle-level professional titles respectively; the awareness rates of AMI ECG was 50.8%~98.5% and 27.8%~100.0% in the medical workers with primary and middle-level professional titles respectively.(2)the correct rates of reading ECG and operating ECG were 49.2% and 55.6%, 75.4% and 94.4% in the medical workers with primary and middle-level professional titles respectively. With a significant difference before and after the treatment(P<0.05).Conclusion It is necessary to improve the ECG knowledge and the operation of ECG for community medical workers, and training of ECG skill training can help them improve the pre-hospital diagnosis of AMI.
