不同材料在耳内镜下鼓膜修补术中的应用效果比较

摘要:
[摘要] 目的 比较不同材料在耳内镜下鼓膜修补术中的应用效果。方法 选择2020年1月至2021年12月在泉州市第一医院行耳内镜下鼓膜修补术的鼓膜穿孔患者71例(71耳),根据所用修补材料不同将其分为颞肌筋膜组(20例)、耳屏软骨组(41例)和耳屏软骨膜组(10例)。比较三组手术时间、住院时间,以及术后听力恢复情况和鼓膜愈合情况。结果 三组术后平均气导听阈、平均气骨导差缩小值及手术时间比较差异无统计学意义(P>0.05)。三组术后住院时间比较差异有统计学意义(P<0.05),以颞肌筋膜组住院时间最长。颞肌筋膜组、耳屏软骨组、耳屏软骨膜组术后分别有2例(10.00%,2/20)、0例(0.00%,0/41)、3例(30.00%,3/10)发生鼓膜再穿孔,差异有统计学意义(χ2=11.425,P=0.003)。结论 采用耳屏软骨进行耳内镜下鼓膜修补的术后效果好,可降低术后再穿孔率,手术操作较方便,易于推广。

Abstract:
[Abstract] Objective To compare the application effects of different materials in eardrum repair under otoendoscope. Methods Seventy-one patients(71 ears) with tympanic membrane perforation who underwent tympanic membrane repair under otoendoscope in Quanzhou First Hospital from January 2020 to December 2021 were selected. According to the different materials used for eardrum repair, the patients were divided into the temporal muscle fascia group(20 cases), the tragus cartilage group(41 cases) and the perichondrium of tragus group(10 cases). The operation time, the length of hospital stay, the postoperative hearing recovery and tympanic membrane healing were compared among the three groups. Results There were no significant differences in the mean threshold of air conductance, the reduction value of mean air bone conductance and the operation time among the three groups(P>0.05). There was statistically significant difference in the length of hospital stay among the three groups(P<0.05), and the temporal muscle fascia group had the longest hospital stay. Re-perforated tympanic membrane occurred in 2 cases(10.00%, 2/20), 0 cases(0.00%, 0/41) and 3 cases(30.00%, 3/10) in the temporal muscle fascia group, the tragus cartilage group and the perichondrium of tragus group, respectively, with a statistically significant difference among the three groups(χ2=11.425, P=0.003). Conclusion The application of tragus cartilage has a good postoperative effect on eardrum repair under otoendoscope, which can reduce the rate of postoperative re-perforation, and the surgery is easy to operate and extend the application.
