女性生育力保存的技术选择策略与适应证:现状、挑战与未来展望

摘要:
[摘要] 随着恶性肿瘤患者生存率提高、社会婚育年龄普遍推迟以及免疫、代谢、遗传性疾病诊疗进展,女性生育力保存已发展为生殖医学与肿瘤学、血液学、免疫学、代谢学、临床遗传学等多学科交叉的重要临床领域。目前,已形成以胚胎冷冻、卵母细胞冷冻和卵巢组织冷冻与移植(OTCT)为核心的女性生育力保存技术体系。该文基于国内外权威指南与循证医学证据,系统梳理女性生育力保存技术的临床应用现状与特征,深入解读从疾病相关指征到社会性需求的适应证谱系,构建“人群分层-需求导向-技术适配”的个体化选择策略。同时,剖析当前面临的生育力保存技术争议、临床可及性不足、多学科协作机制不完善及伦理、法律困境等核心挑战,并对未来技术创新、体系优化与政策支持方向进行展望,旨在为生育力保存的临床规范化与精准化决策提供参考,推动我国女性生育力保存事业健康发展。

Technical selection strategies and indications of female fertility preservation: current status, challenges and future perspectives

Abstract:
[Abstract] With the increasing survival rate of malignant tumor patients, the overall trend toward delayed marriage and childbearing, and advances in the diagnosis and treatment of immune, metabolic, and genetic diseases, female fertility preservation(FP) has emerged as a pivotal interdisciplinary field spanning reproductive medicine, oncology, hematology, immunology, metabology and clinical genetics. At present, a technical system for female FP has been established, with embryo cryopreservation, oocyte cryopreservation and ovarian tissue cryopreservation and transplantation(OTCT) as the core techniques. Based on authoritative domestic and international guidelines and evidence-based medical researches, this paper systematically sorts out the current clinical application status and characteristics of female FP techniques, deeply interprets the indication spectrum ranging from disease-related indications to social needs, and constructs an individualized selection strategy of “population stratification-demand orientation-technique adaptation”. Meanwhile, this paper analyzes the core challenges currently faced, such as technical disputes in FP, limited clinical accessibility, insufficient multidisciplinary team(MDT) coordination, and ethical and legal dilemmas. It also provides an outlook on future technological innovations, system optimization, and policy support directions, aiming to provide reference for the clinical standardization and precision decision-making of FP and promote the healthy development of the cause of female FP in China.
