彩色多普勒超声联合血清GP73、MUC2鉴别乳腺肿块良恶性的价值

摘要:
[摘要] 目的 探讨彩色多普勒超声(CDU)联合血清高尔基蛋白73(GP73)、黏蛋白2(MUC2)鉴别乳腺肿块良恶性的价值。方法 选取2022年2月至2024年2月河北省直属机关第二门诊部收治并接受手术治疗的132例女性乳腺肿瘤患者。根据术后病理结果将患者分为良性病变组(53例)和乳腺癌组(79例)。所有患者术前均完成CDU检查。比较两组肿瘤内的搏动指数(PI)、最大血流速度(Vmax)、血流阻力指数(RI)。采用酶联免疫吸附试验(ELISA)测定血清GP73、MUC2水平。通过受试者工作特征(ROC)曲线分析血清GP73、MUC2水平对乳腺癌的诊断价值。采用Kappa一致性分析评估CDU、血清GP73、血清MUC2及其联合诊断结果与病理诊断结果的一致性。结果 与良性病变组比较,乳腺癌组乳腺肿块形态不规则占比、边界模糊占比、有血流占比、PI、RI、Vmax较高,血清GP73、MUC2水平较高,差异有统计学意义(P<0.05)。ROC曲线分析结果显示,血清GP73、MUC2水平均可有效诊断乳腺癌(P<0.05)。CDU、血清GP73、血清MUC2单独诊断与病理诊断的一致性分析显示,Kappa值分别为0.710、0.517、0.607(P均<0.05)。CDU与血清GP73、MUC2联合诊断与病理诊断的一致性分析显示,Kappa值为0.858(P<0.05)。CDU联合血清GP73、MUC2诊断乳腺癌的灵敏度、准确度、阴性预测值均显著高于单一指标(P<0.05)。结论 乳腺癌患者血清GP73、MUC2水平升高,CDU联合血清GP73、MUC2诊断乳腺癌的灵敏度、准确度、阴性预测值较高,有助于提高乳腺癌的早期检出率。

Abstract:
[Abstract] Objective To explore the value of color Doppler ultrasound(CDU) combined with the detection of serum Golgi protein 73(GP73) and mucin 2(MUC2) in differentiating benign and malignant breast masses. Methods A total of 132 female patients with breast tumors who were admitted to the Second Outpatient Department of the Directly Subordinate Agency of Hebei Provincial Government and underwent surgical treatment from February 2022 to February 2024 were selected. According to the postoperative pathological results of the patients, they were divided into benign breast lesion group(53 patients) and breast cancer group(79 patients). All the patients underwent CDU examination before surgery. The pulsatility index(PI), maximum blood flow velocity(Vmax) and blood flow resistance index(RI) in the tumors were compared between the two groups. The levels of serum GP73 and MUC2 were determined by using enzyme-linked immunosorbent assay(ELISA). The diagnostic value of serum GP73 and MUC2 levels for breast cancer was analyzed by using receiver operating characteristic(ROC) curve. Kappa identity test was used to evaluate the consistency of the diagnostic results by using CDU, serum GP73, serum MUC2 and their combination with the pathological diagnostic results. Results Compared with the benign breast lesion group, the breast cancer group had higher proportions of breast masses with irregular shapes, blurred boundaries and blood flow, and higher PI, RI and Vmax, and higher levels of serum GP73 and MUC2, with statistically significant differences between the two groups(P<0.05). The results of ROC curve analysis showed that both serum GP73 and MUC2 levels could effectively diagnose breast cancer(P<0.05). The analysis on the consistency of the diagnostic results by using CDU, serum GP73 and serum MUC2 alone with the pathological diagnostic results showed that their Kappa values were 0.710, 0.517 and 0.607, respectively(P<0.05). The analysis on the consistency of the diagnostic results by using CDU combined with serum GP73 and MUC2 with the pathological diagnostic results showed that the Kappa value was 0.858(P<0.05). The sensitivity, accuracy and negative predictive value of CDU combined with serum GP73 and MUC2 in diagnosing breast cancer were significantly higher than those of a single indicator(P<0.05). Conclusion The levels of serum GP73 and MUC2 in breast cancer patients are elevated, and the sensitivity, accuracy and negative predictive value of CDU combined with serum GP73 and MUC2 in diagnosing breast cancer are relatively high, which help to improve the early detection rate of breast cancer.
