血清miRNA-210、miRNA-124及miRNA-195水平与脑小血管病患者磁共振成像总负荷评分和认知功能的关系研究

摘要:
[摘要] 目的 研究血清miRNA-210、miRNA-124及miRNA-195水平与脑小血管病(CSVD)患者磁共振成像(MRI)总负荷评分和认知功能的关系。方法 选择2023年3月至2024年12月宁德师范学院附属宁德市医院收治的90例CSVD患者作为研究组。另选择本院同期30名体检健康者作为对照组。采用实时荧光定量聚合酶链反应(RT-qPCR)检测血清miRNA-210、miRNA-124及miRNA-195水平。研究组接受MRI检查,根据MRI总负荷评分将患者分为轻度负荷组(0分结果 研究组miRNA-210、miRNA-124及miRNA-195水平显著低于对照组(P<0.05)。中重度负荷组miRNA-210、miRNA-124及miRNA-195水平显著低于轻度负荷组(P<0.05)。认知功能障碍组miRNA-210、miRNA-124及miRNA-195水平显著低于认知功能正常组(P<0.05)。Spearman秩相关分析结果显示,miRNA-210、miRNA-124及miRNA-195水平与MRI总负荷评分呈负相关(rs分别为-0.529、-0.692、-0.536,P均<0.001),与MMSE评分呈正相关(rs分别为0.659、0.672、0.620,P均<0.001)。多因素logistic回归分析结果显示,较高的miRNA-210、miRNA-124、miRNA-195水平是抑制CSVD患者发生认知功能障碍的保护因素(P<0.05)。ROC曲线分析结果显示,miRNA-210、miRNA-124及miRNA-195均可有效诊断CSVD患者认知功能障碍(P<0.05),三者联合的诊断效能[AUC(95%CI)=0.964(0.902~0.992)]较单一指标更高(P<0.001),灵敏度为96.77%,特异度为78.14%。结论 CSVD患者血清miRNA-210、miRNA-124及miRNA-195水平明显下调,与MRI总负荷评分及认知功能显著相关,三者联合可有效诊断CSVD患者认知功能障碍。

Abstract:
[Abstract] Objective To study the associations of serum microRNA-210(miRNA-210), microRNA-124(miRNA-124) and microRNA-195(miRNA-195) levels with magnetic resonance imaging(MRI) total load scores and cognitive function in cerebral small vessel disease(CSVD) patients. Methods A total of 90 patients with CSVD who were admitted to Ningde Municipal Hospital of Ningde Normal University from March 2023 to December 2024 were selected as research group. Thirty healthy individuals who received physical examination in the hospital during the same period were selected as control group. The levels of serum miRNA-210, miRNA-124 and miRNA-195 were detected by using real-time fluorescence quantitative polymerase chain reaction(RT-qPCR). The research group received MRI examination. According to MRI total load scores, the patients were divided into the mild load group(0 pointsResults The levels of miRNA-210, miRNA-124 and miRNA-195 in the research group were significantly lower than those in the control group(P<0.05). The levels of miRNA-210, miRNA-124 and miRNA-195 in the moderate-to-severe load group were significantly lower than those in the mild load group(P<0.05). The levels of miRNA-210, miRNA-124 and miRNA-195 in the cognitive dysfunction group were significantly lower than those in the normal cognitive function group(P<0.05). The results of Spearman rank correlation analysis showed that the levels of miRNA-210, miRNA-124 and miRNA-195 were negatively correlated with the MRI total load scores[Their correlation coefficients(rs) were -0.529, -0.692 and -0.536, respectively, P<0.001], and were positively correlated with the MMSE scores(Their rs were 0.659, 0.672 and 0.620, respectively, P<0.001). The results of multivariate logistic regression analysis showed that higher levels of miRNA-210, miRNA-124 and miRNA-195 were protective factors for inhibiting cognitive dysfunction in the CSVD patients(P<0.05). The results of ROC curve analysis showed that miRNA-210, miRNA-124 and miRNA-195 could effectively diagnose cognitive dysfunction in the CSVD patients(P<0.05), and the combined diagnostic efficacy of the three indicators[AUC(95%CI)=0.964(0.902-0.992)] was higher than that of a single indicator(P<0.001), with a sensitivity of 96.77% and a specificity of 78.14%. Conclusion The levels of serum miRNA-210, miRNA-124 and miRNA-195 in CSVD patients are significantly down-regulated, which are significantly correlated with MRI total load scores and cognitive function. The combined detection of the three indicators can effectively diagnose cognitive dysfunction in the CSVD patients.
