冷凝集对血细胞分析结果的影响及消除方法探讨

摘要:
[摘要] 目的 探讨冷凝集对血细胞分析结果的影响以及温水浴、置换血浆两种不同处理方法对该现象的消除效果。方法 将该院收集的4例出现不同程度冷凝集现象的标本,分别进行温水浴和置换血浆两种方法进行处理,并用全自动血细胞分析仪在标本处理前后分别进行检测,观察其检测结果的变化。结果 2例标本在接受温水浴处理之后其冷凝集状况没有得到全部消除,但经置换血浆处理之后冷凝集现象得到了完全消除;另2例标本在同时接受温水浴和置换血浆两种方法处理之后其冷凝集状况均得到了全部消除.置换血浆结合温水浴去除法所检到的白细胞(WBC)、平均红细胞容积(MCV)、平均红细胞血红蛋白(MCH)和血小板(PLT)均低于处理前的各项指标的值,红细胞(RBC)高于处理前结果;而单独用置换血浆法所检测到的WBC、MCV、MCH和PLT值均低于温水浴所得结果,RBC则高于温水浴法。结论 使用血浆置换法不仅能够消除冷凝集现象,而且能保证血细胞分析结果不受影响。

Abstract:
[Abstract] Objective To study the effect of cold agglutination on the hematology analysis and the effects of warm water bath and plasma replacement on the elimination of cold agglutination.Methods Four blood samples with different degrees of cold agglutination were collected in our hospital as the research subjects. The agglutinated blood samples were treated with a warm water bath or plasma replacement respectively and then subjected to hematology analysis using an automatic hematological analyzer. The detective results were compared between the two treated methods for cold agglutination of blood samples.Results The cold agglutination phenomena in samples 1 and 2 were not completely eliminated after warm water bathing, however, the agglutination was completely eliminated after the additional treatment with plasma replacement. The cold agglutination phenomena in samples 3 and 4 were completely eliminated after the treatment with warm water bathing and plasma replacement.The WBC count, MCV, MCH, and platelet count were decreased while the WBC count was increased in the samples treated with the addtion of plasma replacement compared with those before the treatment and those in the samples treated with warm water bathing only.Conclusion Plasma replacement is a better way to eliminate the cold agglutination phenomena in hematology analysis and to ensure the accuracy of hematological analysis of peripheral blood in a clinical laboratory.
