摘要:
[摘要] 随着恶性肿瘤治疗技术的进步和年轻癌症患者生存率的显著提高,以及社会因素导致的生育年龄推迟,生育力保存已成为生殖医学领域至关重要的组成部分。该文旨在从实验室核心技术视角,系统梳理当前生育力保存技术的发展现状及未来趋势。文章重点剖析以玻璃化冷冻为金标准的配子(精子、卵母细胞)与胚胎保存技术的原理、标准化流程及效能评估体系,深入探讨卵巢组织与睾丸组织等性腺组织冷冻保存面临的挑战与复苏策略,并评述未成熟卵母细胞体外成熟培养(IVM)、体外卵泡培养(IVFC)以及干细胞技术与体外配子发生(IVG)等前沿实验室策略的潜力与瓶颈。最后,对生育力保存技术向自动化、智能化及个体化精准发展的方向进行展望。

Research progress in laboratory techniques for fertility preservation

Abstract:
[Abstract] With the advancement of treatment techniques for malignant tumors and a significant increase in survival rate of young cancer patients, as well as the delay in childbearing age due to social factors, fertility preservation(FP) has become a crucial component in the field of reproductive medicine. This paper aims to systematically sort out the current development status and future trends of FP from the perspective of core laboratory techniques. It focuses on analyzing the principles, standardized procedures, and efficacy evaluation systems of gametes(sperms and oocytes) and embryo cryopreservation using vitrification as the gold standard, and delves into the challenges and post-thaw strategies associated with the cryopreservation of gonadal tissues, such as ovarian tissues and testicular tissues, and makes comments on the potential and bottlenecks of cutting-edge laboratory strategies, including in vitro maturation(IVM) of immature oocytes, in vitro follicle culture(IVFC), and stem cell-based in vitro gametogenesis(IVG). Finally, this paper provides an outlook on the development of FP techniques towards automation, intelligence, and individualized precision.
